Free field trip permission slip template
Parent permission for a student to attend a school trip.
Field trips fail when three slips are still in backpacks. An online permission form collects destination, date, parent contacts, and a signature before the bus leaves.
Use this for single-day outings. Ongoing clubs and programs should also keep a general parental consent on file. Overnight or out-of-state travel may need the minor travel consent letter as well.
Send the link in the class email and print a QR for pickup. Schools hub covers the morning-of checklist. Not legal advice — follow district-required language when they publish it.

Questions
Parental consent or field-trip permission?
Parental consent covers season-long participation. Field-trip permission adds the destination and date for a specific outing. Many programs use both.
Can Jarvis match our district form?
Yes. Photograph the required sheet and ask Jarvis to draft from it, then have your admin review before publishing.
What about sports away games?
Use a sports waiver for athletics risk language, and field-trip or travel consent when transportation and overnight stays are involved.
