Free sports waiver template

Release of liability and assumption of risk for sports practices, games and training.

Sports waivers record that athletes (or their parents) understood contact, overuse, and facility risks and still chose to play. Name the sport and the season — generic 'athletics' is weaker.

Youth teams almost always need a parent signature. Schools and camps often pair this with parental consent or field-trip permission for travel games.

Send the link with registration. Coaches should not be collecting wet-ink sheets at the first practice. Not legal advice.

Questions

Does a parent sign for youth players?

Usually yes. Keep the minor participant name on the form and a parent signature. Enforceability varies by state.

Away games and travel?

Add field-trip permission or minor travel consent when transportation or overnight stays are involved.

One waiver per season or per game?

Most leagues collect once per season. Update and re-collect if the published form text changes materially.
